How to Register
You can either visit the surgery to collect an application form which will include a short questionnaire or download the forms from here.
Please check with our our reception team for a list of doctors accepting new patients. Please indicate on the New Patient Registration Form which Doctor you would like to register with. If you do not specify a doctor, we can allocate a doctor to you.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct
